Membership

Membership is free and open to qualified individuals. PVA's President invites you to join our more than 19,000 members today and have your voice, energy and efforts assist others living with SCI/D.
By joining the PVA North Central Chapter, you have the opportunity to have your voice, your energy and your efforts assist others. Upon joining you will begin to receive the chapter's monthly newsletter, as well as notifications of upcoming events and meetings. In addition, you will have the opportunity to participate in many programs, including volunteering and sporting events.
The regular general membership meetings are held on the second Thursday of each month at 1:00 p.m. at the Chapter Office. The Chapter Board of Directors meets every other month to discuss the general business of the organization.
There are two categories for membership: Regular and Associate

Gravesite or Columbarium Niche
Pre-placed Grave liner (Standard Size and some Oversized)
Headstone/Niche Cover
Perpetual Care

Costs:

Veteran
There is no fee for burial of an eligible veteran.

Spouse/Eligible Dependent
$796 (all burial options)

Additional charges may be added if the casket is oversized and a larger grave-liner is needed.

If a grave-liner is supplied as part of a prepaid burial plan, the plan will be reviewed and every effort made to meet the terms of the prepaid plan within existing cemetery guidelines.

** Fees for spouse/eligible dependents cannot be prepaid and are subject to change.

1. Visit www.fordpartner.com and enter your Partner Code – (Contact PVA Membership for
access code @ 1-800-424-8200)
2. Select the “PINs” tab from the top navigation
3. Enter the last four digits of your Social Security (or Social Insurance) Number; and your
name, address, city, state (or province) and ZIP (or postal) code to obtain your PIN
4. Print your PIN Certificate and bring it along when visit a participating dealer
5. Identify yourself as a X-Plan Partner Recognition participant, and provide the last four
digits of your Social Security (or Social Insurance) Number and your PIN to confirm
eligibility
6. Provide proof of employment or organization membership with a recent organization I.D.
card, pay stub, W-2 (or T4) form or health care card
7. Select an X-Plan eligible vehicle and ask for a copy of the invoice to confirm X-Plan

8. Complete the purchase or lease agreement and arrange for delivery
9. Sign the AXZD-Plans Pricing Agreement and Customer Cash Payment Authorization form

You would just need to present a valid government issued military ID such as a Veteran's Identification Card, a Uniformed Services Identification Card, or a Common Access Card to the cashier at the time of purchase.
